Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Advanced Energy, LLC at Weld County Road 10 & 43, Hudson, CO 80642: On and before April 10, 2015, employees working from the operator's platforms were not protected from crushing hazards in that the drill operators on a Walker-Neer 250-40 Truck Mounted Rig were not adequately protected from the shearing action caused by the steel platform on the pipe handling boom when the boom cantilevers to the down position and ultimately rests adjacent to the operator's platform. This condition exposed employees to a crushing hazard and resulted in an employee receiving a severe foot injury.
