29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) 10 Puddingstone Way, Warren, NJ 07059. Employees were exposed to head injuries from falling debris and or equipment when working under overhead roofing activities while not wearing any head protection, i.e. hard hats.    Condition occurred on or about 04/21/15.

29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:   a) 10 Puddingstone Way, Warren, NJ 07059. Employees were exposed to eye and face injuries from flying debris when operating a pneumatic nail gun while not wearing any eye and or face protection, i.e. safety glasses.    Condition occurred on or about 04/21/15.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 10 Puddingstone Way, Warren, NJ 07059. Employees were exposed to fall hazards when they were placing and installing new roofing materials with no fall protection systems in place.     Condition occurred on or about 04/21/15.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:   a) 10 Puddingstone Way, Warren, NJ 07059. Employees were exposed to fall hazards when using a portable extension ladder to access the roof when the ladder was damaged and defective in that it was missing both safety shoes. The ladder was not tagged Do Not Use and or taken out of service.  Condition occurred on or about 04/21/15.
