29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a) Pallet Assembly: An employee, working at the Duomatic Nailer, was exposed to continuous noise at 136.0% of the exposure level of 90 dBA or an 8-hour time-weighted average of 92.2 dBA. This exposure was observed over a 418 minute sampling period. Zero exposure was assumed for the 62 minutes not sampled. The employer did not establish and maintain an audiometric testing program for those employees exposed over the action level of 85 dBA, on or about 05/21/2015. b) Pallet Assembly: An employee, working at the Turbo Nailer, was exposed to continuous noise at 128.4% of the exposure level of 90 dBA or an 8-hour time-weighted average of 91.8 dBA. This exposure was observed over a 423 minute sampling period. Zero exposure was assumed for the 57 minutes not sampled. The employer did not establish and maintain an audiometric testing program for those employees exposed over the action level of 85 dBA, on or about 05/21/2015. c) Sawmill: An employee, working at the Pendu Resaw, was exposed to continuous noise at 242.0% of the exposure level of 90 dBA or an 8-hour time-weighted average of 96.4 dBA. This exposure was observed over a 377 minute sampling period. Zero exposure was assumed for the 103 minutes not sampled. The employer did not establish and maintain an audiometric testing program for those employees exposed over the action level of 85 dBA, on or about 05/21/2015. d) Sawmill: An employee, working at the Pendu Resaw, was exposed to continuous noise at 287.0% of the exposure level of 90 dBA or an 8-hour time-weighted average of 97.6 dBA. This exposure was observed over a 368 minute sampling period. Zero exposure was assumed for the 112 minutes not sampled. The employer did not establish and maintain an audiometric testing program for those employees exposed over the action level of 85 dBA, on or about 05/21/2015. e) Sawmill: An employee, working at the Slab Recovery Stacker, was exposed to continuous noise at 142.8% of the exposure level of 90 dBA or an 8-hour time-weighted average of 92.6 dBA. This exposure was observed over a 418 minute sampling period. Zero exposure was assumed for the 99 minutes not sampled.
