29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a) At the work site, a pump jack scaffold was not inspected prior to use by a competent person in that the designated competent person did not know that the vertical poles were required to be secured to the structure at the top and bottom; that fall protection was required to be provided in the form of either a full guardrail system or fall arrest system; and that any bracket, braces, or accessories were required to be fabricated from metal plates and angles.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  a) At the site, on the north side, a pump jack scaffold was erected without the oversight of a competent person in that the competent person overseeing the erection of the scaffold did not know that the vertical poles were required to be secured to the structure at the top and bottom; that fall protection was required to be provided in the form of either a full guardrail system or fall arrest system; and that any bracket, braces, or accessories were required to be fabricated from metal plates and angles.  b) At the site, on the south side, a pump jack scaffold was erected without the oversight of a competent person in that the competent person overseeing the erection of the scaffold did not know that the vertical poles were required to be secured to the structure at the top and bottom; that fall protection was required to be provided in the form of either a full guardrail system or fall arrest system; and that any bracket, braces, or accessories were required to be fabricated from metal plates and angles.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a) At the work site, an employee was exposed to fall hazards of approximately 18 feet while performing siding work without fall protection.
