29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employee turned off the equipment or machine: (a) On or about 4/16/15, in the Welding Department, the energy control procedures for the Hinged Plate Robotic Welder, Monorail Robotic Welder, and Booth Welders did not adequately address preparation for shutdown in that the authorized employee did not have knowledge of the magnitude of the argon/oxygen gas energy.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: (a) On or about 4/16/15, in the Plastics Area, the employer exposed employees to amputation hazards, in that the react/radial saw blade used was a smaller diameter than the size recommended by the manufacturer causing the guard at the lower portion of the blade to be ineffective, while cutting plastic pipe.
