29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) JBB Drywall, 85 Sunshine Circle, Erie, CO 80516: On and before April 13, 2015 the exposing employer did not ensure the working platform on the fabricated frame scaffold was fully planked in that only 2 of 3 planks were placed on the working level of the scaffold. This condition exposed the employees to a fall of approximately 6 feet.

Without the inspections employees were exposed to fall hazards of approximately 6 feet.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secured vertical members together laterally: (a) JBB Drywall, 85 Sunshine Circle, Erie, CO 80516: On and before April 13, 2015 the exposing employer did not ensure all brace connections were secured on the fabricated frame scaffold in use, in that cross braces on one side were missing. The lack of cross braces exposed employees to the hazard of falling approximately 6 feet due to an unsteady and weakened scaffold.
