29 CFR 1910.23(a)(2): Ladderway floor openings or platforms were not guarded by a standard railing with standard toeboards on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: On or about June 18, 2015, in the snap chill area, employees were allowed to work from an elevated platform with inadequate railing, exposing the employees to fall hazards.

On or about April 10, 2015, in valve alley, employees were not safely evacuated from the facility during an ammonia leak.
