29 CFR 1910.178(l)(4)(i): The employer did not conduct refresher training, including an evaluation of the effectiveness of that training, as required by paragraph (l)(4)(ii) to ensure that the operators has the knowledge and skills needed to operate the powered industrial truck safely.  On or about April 22, 2015, employees were not operating powered industrial trucks safely and the employer failed to conduct refresher training including an evaluation of the effectiveness of that training.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled:    (a) Throughout the facility's mail handling areas - on or about April 22, 2015, employees were permitted to use forklifts to push multiple (up to 10) palletized mail loads on the ground, exposing other employees to struck-by hazards capable of causing serious injuries.     (b) Throughout the facility's mail handling areas - on or about April 22, 2015 employees were permitted to pull numerous (up to 20) "all purpose containers" and (up to 7) "bulk mail containers" exposing other employees to struck-by hazards capable of causing serious injuries.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).    The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(o)(1), which was contained in OSHA inspection number 891598, citation number 01, item number 002 and was affirmed as a final order on March 21, 2014, with respect to a workplace located at 5800 Phantom Drive, Hazelwood, MO 63042.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)    Toyota powered industrial truck no. F17: on or about April 22, 2015, employee(s) were operating a forklift with a steering column in need of repair where it could not be secured/locked in place, and was tied with rubber bands to prevent movement.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
