29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about April 13, 2015, at buildings No.16 and 24, employees using air nail guns to do framing work were not wearing eye protection, exposing themselves to being struck by flying debris or nails.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a. On or about April 13, 2015, at buildings No.16 and 24, employees stepped on the top step of a 6-foot Werner stepladder to access the second level to frame floor joists, exposing themselves to a fall hazard.  b. On or about April 13, 2015, at the north side of building No.24, one employee stepped on the top step of a 6-foot Werner stepladder to do framing work and was exposed to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. Employees performing framing work on elevated levels 6 feet or more above ground are not protected from 12ft-8in fall hazard by a fall protection system.  This violation was most recently observed on or about April 13, 2015, at a job site located at 100 Brannan Rd, Saint Johns, Florida.  O.P. CCC, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 976052, citation 01, item 1, issued on June 4, 2014, with respect to a workplace at 6808 Goldflower Ave., Harmony, FL, 34773.  The final order date was July 14, 2014.  O.P. Construction Contractors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 933668, citation 01, item 3, issued on September 9, 2013, with respect to a workplace at 792 Sunny Stroll Dr., Middleburg, FL, 32050.  The final order date was September 25, 2013.  O.P. Construction Contractors, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 561619, citation 01, item 1, issued on August 28, 2012, with respect to a workplace at 2031 Lynch, Site 238, Apopka, FL, 32703.  The final order date was September 13, 2012.
