29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) North Plant-Section Number 2 - Blender/Finish Grinder 3: On or about April 15, 2015 employees performing sanitation cleaning of blender did not deenergize the blender by locking out the energy sources. This exposed employees to crushing, caught-in and amputation hazards. In The Alternative: 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) North Plant-Section Number 2 - Blender/Finish Grinder 3: On or about April 15, 2015 employees performing sanitation cleaning of blender and grinder did not place guarding over the discharge opening of the blender during the agitation cleaning step of the sanitation process. This exposed sanitation personnel to crushing, caught-in and amputation hazards.
