29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5.  a) On or about June 2, 2015 in the Electrical Testing area; employer failed to protect employees involved in operating electrical switches, gearing and other energized equipment from a shock hazard when the protective gloves ( Salisbury Class 2 - size 9.5 ) were issued in July 2014 and last testing occurred on April 30, 2014. 29 CFR 1910.157(g)(4): Employees who have been designated to use fire fighting equipment as a part of the emergency action plan were not provided training in the use of appropriate equipment upon initial assignment and at least annually thereafter:  a) On or about 06/02/15 throughout the facility; employer failed to protect employees designated to use fire fighting equipment during activities, such as but not limited to, transferring flammable liquids, using a cutting torch, exposing them to fire hazards. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.  a) On or about 06/02/15 in the Fabrication area; employer failed to protect employees who operate the Cincinnati brand press brake ( 30 ton - serial # 49750, 10 ft ) from caught between hazards during the bending and shaping of various parts.
