29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:    a) Construction site, addition north of Macy's - On or about April 13, 2015, an employee walking/working on the site wore shoes that did not provide appropriate protection for the top and sides of his feet from various hazards such as heavy concrete pump lines, motorized equipment, projections and sharp edges of structural steel construction. The shoes did not have appropriate soles to provide traction on wet or slippery surfaces, or robust soles and uppers to provide protection from penetration from screws, nails, or other items of debris commonly observed on the ground of a construction site.
