29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.   (a)  Assembly:  Employees were exposed to metal or chemicals, such as propane and denatured alcohol, which could be detrimental to their eyes. Employees with prescription lenses were not wearing safety glasses with prescription lenses with side shields, or other eye protection over the prescription lenses.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  (a) Production:  Forklift Operators were exposed to burns of the skin as appropriate hand protection was not required when employees were changing propane tanks for the forklifts.  On or about April 15, 2015, an employee received burns to his hands when appropriate hand protection was not required and the employee came into contact with propane gas.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  (a)  Assembly: Employees were exposed to hazards of being struck by a forklift, as the TCM  Pro CG 25 Propane Forklift had rear tires that had cracks, and a left rear brake light that was out and was not taken out of service until repaired.  (b) Assembly:  Employees were exposed to hazards of being struck by a forklift as the TCM T-3114 Propane Forklift had a left rear brake light that was out and was not taken out of service until repaired.

Safety data sheets were not readily accessible to the employees as there was no book with copies of safety data sheets, and the computer was not set up where employees could access safety data sheets online.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Assembly:  Employees were exposed to chemical burns as employees were not trained on hazardous chemicals at the time of initial assignment and whenever a new chemical is introduced into their work area.  Employees did not have initial chemical training for chemicals such as, propane, and denatured alcohol.
