29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     a)  At 101 Hickory Street in Farmington, MO - On or about 04/22/2015, employees were working on a residential roof (steep roof) approximately 12ft above a lower solid concrete drive-way and sidewalk without having any fall protection system in place, exposing employees to elevated fall hazards.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     a)  At 101 Hickory Street in Farmington, MO - On or about 04/22/2015, employees were working on a residential roof (steep roof) approximately 12ft above a lower solid concrete drive-way and sidewalk without having any fall protection system in place and without receiving effective training to enable each employee to recognize hazards of falling, exposing the employees to elevated fall hazards.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally:    a)  At 101 Hickory Street in Farmington, MO - On or about 04/22/2015, employees were using a "Werner" 24-ft aluminum extension ladder to access a residential roof (steep roof), approximately 12-ft above a lower earth surface, where the ladder side rails were placed at the rake portion of the roof, creating uneven support and elevated fall hazards.
