29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: At the south wall of a commercial building job site, the scaffold legs of a fabricated frame scaffold were placed on strips of scrap lumber not adequate to support a scaffold three tiers high. Employees were exposed to scaffold tip overs while applying stucco to the wall.

29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress. At the south wall of a commercial building jobsite, employees climbed scaffold braces up and down to gain access to the work platform and to climb down from the scaffold. Employees were exposed to falls greater than 10 feet.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The endpoints at each upright were more than 48 inches (1.3 m) apart. At the south wall of a commercial building jobsite, employees worked from a scaffold platform two and three tiers high without midrails.
