29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a. 270 Fdc By-products Drive, Eastanollee, GA (Evaporator Area) - Employees were exposed to fall hazard. Employees were required to access the walking/working (catwalk) surface to open and closed the water valve. The catwalk platform was not equipped with a standard guardrail system at the entrance point, on or about 04/22/15.

29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a. 270 Fdc By-products Drive, Eastanollee, GA (Jobsite) - Employees were exposed to fall and overturning hazard. Employees (maintenance) constructed an attachment (man-lift basket) to go on the forks of Toyota forklift. The lift basket is used to lift employees in areas where there are no access and/or catwalk (walking/working surface) to lubricate the gear box. The man lift attachment was not approved by the forklift manufacturer and/or a qualified register engineer, on or about 04/22/15.
