Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a)Block 703, Lot 12 with a postal address of 220 Route 73 South in Berlin, NJ- Rear roof: Employees engaged in commercial roofing activities without means of fall protection or safety monitor at a height of approximately 12 feet, observed on or about 4/22/15. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
