29 CFR 1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person were not maintained. When that ability is interrupted at any time, the operator did not safely stop operations requiring signals until it is reestablished and a proper signal is given and understood: On or about 04/20/15, the signal man/iron worker and the crane operator did not maintain communication at all times when the beam was being moved and lowered for placement. Employees were exposed to stuck by hazards.
