29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Meat Cutting Area- The employer failed to ensure that all areas where hazards created by the point of operation were guarded on machinery such as the Hollymatic Patty Maker, Super Model 54 Food Portioning Machine, Serial Number 65778, to prevent injury to employees during the machine's operation.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures were not weatherproof in wet locations:    a) Meat Cutting Area- The employer failed to ensure all boxes and fittings in damp or wet locations, such as the junction box on the Hobart 6801 Meat Cutting Saw, Serial Number 31-1446297, and the Hobart 6801 Meat Cutting Saw, Serial Number 31-1446293, at the time of the inspection did not prevent moisture and water from entering and accumulating.    In addition, the box on the saw with the Serial Number 31-1446297 did not have a tight connection where the service cord exited the box, which could increase the likelihood of water accumulation within the box.  The boxes on both of the saws showed signs of corrosion from the wet environment.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    a) Meat Cutting Area-  The employer failed to ensure that flexible cords, such as the flexible cord supplying 220 volts of power to the Hobart 6801 Meat Cutting Saw, Serial Number 31-1446297, were provided with strain relief at the junction box.  The outer sheath on the flexible cord was missing at the attachment point with the junction box for the saw.        No abatement certification or documentation is required for this item.
