29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   At the construction site, employees working on the ground were not wearing head protection and exposed to head injuries from falling decking material.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   At the construction site, employees installing decking on the roof were not protected from falling approximately 12 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   At the construction site, employees used a portable extension ladder to access the roof area and the side rails did not extend at least three feet above the landing exposing employees to fall hazards.
