29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields:  (a)  Pallet Building Area:  Two employees wore prescription glasses while operating a circular saw, reciprocating saw and chop saw to cut wood used to build and recycle wooden pallets.  The employees' prescription glasses were not impact resistant or equipped with side shields, as observed on April 22, 2015.    Abatement Certification Required for this Item.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  (a)  Facility Wide:  Employees operate a Toyota forklift (serial #12781, model #8FGU15) to move pallets around the facility.  The employer has not ensured that employees received training and were evaluated on the operation of the forklift prior to forklift use, as determined on April 22, 2015. 29 CFR 1910.303(b)(1)(ii):   Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the mechanical strength and durability were not maintained:     (a) Pallet Building Station:  An employee used a Skil Circular Saw that was provided power from an extension cord that was damaged in three places exposing the employee to the energized insulated conductors, as observed on April 22, 2015.  (b)  Pallet Building Station:  An employee used a DeWalt reciprocating saw that was equipped with a damaged power cord exposing the employee to the energized insulated conductors, as observed on April 22, 2015.  Abatement Certification Required for this Item.

29 CFR 1910.1200(e)(1):   The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a) Pyramid Pallet:  Employees use saws to dismantle and rebuild pallets which produce combustible wood dust.  The employer has not developed or implemented a written hazard communication program for employees exposed to combustible wood dust, as determined on April 22, 2015.      ABATEMENT NOTE:  A written program shall include a description of how the criteria for the following will be met:  1.        Labeling and other forms of warnings;  2.        Safety Data Sheets;  3.        Employee information and training.      Additionally, a list of hazardous chemicals known to be present in the workplace must be compiled.  Methods used to inform employees of the hazards associated with non-routine tasks and the informing of contractors of workplace hazards, including a description of the labeling system used in the facility and the availability of safety data sheets must also be addressed.  The written program must be made available upon request.   Abatement Certification Required for this Item.
