29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. On or about April, 22, 2015, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded scaffold, twenty nine (29) feet three (3) inches above the ground, was not provided with an access ladder, thereby exposing the employee to falls.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about, April 22, 2015, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded framed scaffold, twenty nine (29) feet three (3) inches above the ground, was exposed to fall hazards due to the lack of a guardrail system at the ends of the scaffold platform.
