29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a. On or about April 23, 2015, employees who worked from scaffolds while installing stucco and paint were not trained by a scaffold competent person to recognize scaffold hazards such as incomplete planking, inadequate guardrails, and excessive distances between working surfaces and the scaffolds.

29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the assembly of scaffolds: a. On or about April 23, 2015, at the Arium Apartment complex, employees who had not been trained by a scaffold competent person were allowed to reconfigure scaffold components while working from scaffolds to install lathing, stucco, and paint, and these employees were not able to recognize hazards such as incomplete planking, inadequate guardrails, and excessive distance from the front of the scaffold to the working surface.
