OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Employees operated a Skyjack DC Electric scissor lift with model number SJIII 3219 and marking N76240 to perform light removal when the overhead conveyor system transporting aluminum truck boxes began moving. An aluminum truck box struck the scissor lift, causing the lift to tip over and employees to fall to the concrete level below.

Employees performed testing using a multi-meter inside of a 480 VOC energized control panel. The employer did not ensure employees were trained and familiar with safety-related work practices required in accordance with 1910.331 through 1910.335.
