29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) On or about April 23, 2015 at the jobsite workers were performing roofing work on a flat roof with unprotected sides without fall protection equipment and exposed to a fall hazard of approximately 18 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: a) On or about April 23, 2015 at the jobsite an employee was exposed to a fall hazard of greater than 18 feet without using any means of fall protection. He did not receive an adequate training program from his employer concerning fall hazards and the procedures to be followed to minimize these hazards.
