29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section: a) Roll Press Machine, Production floor: On or about April 23, 2015 and at times prior, the employer exposed employees to crushed-by hazards in that employees were allowed to work on the Roll Press without having machine specific procedures for shutting down the machine and insuring that all sources of hazardous energy are isolated, and blocking was not used. b) Thomson Press Machine, Production floor: On or about April 23, 2015 and at times prior, the employer exposed employees to crushed-by hazards in that machine specific procedures did not reflect all sources of energy and how to control each energy source, such as but not limited to; shutdown, locking out, and blocking. c) Laminating Machine, Production floor: On or about April 23, 2015 and at times prior, the employer exposed employees to crushed-by hazards in that machine specific procedures did not reflect all sources of energy and how to control each energy source, such as but not limited to; shutdown, locking out, and blocking. d) Vertical Cutter, Production floor: On or about April 23, 2015 and at times prior, the employer exposed employees to crushed-by hazards in that machine specific procedures did not reflect all sources of energy and how to control each energy source, such as but not limited to shutdown and locking out.
