29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) 12 Mill Road, Morgantown Pennsylvania, Roof - Employees tearing off a roof were exposed to a fall hazard of approximately thirteen (13) feet without the use of an approved method of fall protection on or about April 23rd, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) 12 Mill Road, Morgantown Pennsylvania, Front of House - Employees accessing the roof with a portable step ladder were exposed to a fall hazard of eight (8) feet from the ladder side rails not extending three (3) feet above the working surface, on or about April 23rd, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
