Fayette St., Fayette, Ohio: On or about April 23, 2015, the employer did not ensure an employee's personal fall arrest system was secured (clipped-off) to a Genie #N64530 Aerial Lift. The exposed employee was performing steel erection activities from the aforementioned aerial lift.

29 CFR 1926.760(b)(3): Each employee engaged in a steel erection activity, at heights over 15 and up to 30 feet above a lower level, were not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or be provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section. Fayette St., Fayette, Ohio: On or about April 23, 2015, the employer did not ensure employees were equipped to be tied off while walking approximately 30.00 feet from the North to the South across the rafter, exposed to a fall distance of approximately 26.00 feet. Fayette St., Fayette, Ohio: On or about April 23, 2015, the employer did not ensure employees were equipped to be tied off while walking on the bottom flange of the rafter after exiting the lift, exposed to a fall distance of approximately 23.00 feet.

29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket and were sitting or climbing on the edge of the baskets guardrails or using planks, ladders, or other devices for a work position. A. Henry Gurtzweiler, Inc. worksite located at 705 N. Fayette St., Fayette, Ohio: On or about April 23, 2015, the employer failed to ensure an employee did not stand on the guardrails of a JLG Aerial Lift, model 600S. The employee was exposed to a potential fall due to the working position and using the guardrail to exit the lift to the previously positioned steel framework. Henry Gurtzweiler, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(iv) which was contained in OSHA inspection number 109177, citation number 2, item number 1, and was affirmed as a final order on February 6, 2012,with respect to a workplace located at Oregon, Ohio.
