29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     a) 5030 Woodbridge Avenue, Edison, NJ 08837. Employee was exposed to eye and face injuries from flying debris while operating a pneumatic nail gun when not wearing any eye and or face protection, i.e. safety glasses.      Condition occurred on or about 04/23/15

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:      a) 5030 Woodbridge Avenue, Edison, NJ 08837. Employees were exposed to fall hazards while installing floor joist and sheathing on the second floor of this home with no fall protection system in place.       Condition occurred on or about 04/23/15.

29 CFR 1926.1053(b)(1): Portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder:     a) 5030 Woodbridge Avenue, Edison, NJ 08837. Employee were exposed to fall hazards while using a portable aluminum extension ladder to access the second floor deck when the ladder did not extend three feet above that landing.     Condition occurred on or about 04/21/15.

Employees were exposed to fall hazards when using a six foot fiberglass A frame ladder, not being used as designed and or intended, to access the second floor deck when using the top rung of this ladder as a step.   Condition occurred on or about 04/23/15.
