(b) Sawmill, on or about and at times preceding to April 22, 2015, open pits under out feed/in feed (VDA) vibrating conveyor were not provided standard guarding such as; grating or railings to prevent employee access, exposing employee conducting work activities such as clean up to fall of approximately 2 feet depth x 2 feet length x 1 foot width, resulting in broken bones/fractures.
