29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  On or about 04/23/2015, at 1530 Joy Lane, Decatur, GA; employees of all trades on the construction site were exposed  to building material debris including but not limited to brick, block, wood, metals, and exposed nails that were not properly cleaned up from the ground exposing them to injuries including but not limited to contusions, sprains, punctures, and broken bones. Job tasks on site included but were not limited to work being performed from upper elevations from scaffolding, roofing, and ladders.

29 CFR 1926.451(h)(2)(i): The area below the scaffold was not barricaded, and employees were not prohibited from entering the hazard area.  On or about 4/23/2015, at 1530 Joy Lane, Decatur, GA; employees working on the ground were exposed to struck-by hazards where the working levels on scaffolds had materials such as hand tools, bricks, and blocks on them that could fall and there were no precautions taken to keep employees from working or travelling below the hazards:  a. in the gym, 19 feet, three (3) inch fabricated frame scaffold, b. at the far left and left front of the building, 11 feet, six (6) inch fabricated frame scaffold, and c. at the far right and right front of the building, 28 feet ladder jack scaffold.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  On or about 4/28/2015, in the kitchen area; employees were exposed to fall hazards where two (2) 36 x 36 inch square x seven (7) inches deep holes and five (5) 16 x 16 inch square x six (6) inch deep holes were not filled in a dimly lit room. Employees are required to walk in and around the room to perform plumbing operations behind the block masons.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  On or about 4/23/2015 at 1530 Joy Lane, Decatur, GA; employees were exposed to fall hazards where:  a. an employee was observed exposed to an 11 feet, six (6) inch fall to the lower level while performing measurement operations from a scaffold with no guard rails on the end.  b. employees were exposed to 12 feet, 10 inch fall to the lower level where there were no guard rails installed on the sides of 2nd level of the scaffold in the gym.   c. two (2) laborers were exposed to falls of 12 feet, 10 inches while receiving block from the powered industrial trucks below and placing the block by hand onto the 3rd level where no guard rails were installed on the sides or ends of the scaffold erected to receive block from the ground that was located in the gym in front of the block laying scaffold.   d. two block masons were exposed to 19 feet, three (3) inch fall hazards to the lower level while performing block laying operations from the 3rd level where no guard rails were installed on the sides.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. The frame was used for access.  On or about 4/23/2015, at 1530 Joy Lane, Decatur, GA; employees were exposed to fall hazards   a. of 11 feet, six (6) inches to the lower level where an employee was observed taking measurements from the walk boards of the masons scaffold at the far left of the building.  b. of 19 feet, three (3) inches to the lower level where the ladder did not extend beyond the 3rd level requiring the employees performing block laying operations to scale the frame to get to the 3rd level.
