OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees and that employees were exposed to tripping, caught in between from inoperable lighting: At this well site location, and times there to, a section of the oil rig floor(NW) of the main rig, employees were exposed to slips and tripping hazards along the walk way from inoperable lighting.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. At this oil drilling site, raised flooring was observed by more than 3 inches in the front entrance of NW section of rig floor. Employees are exposed to slip/trip hazards as it is used to access as daily.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. At this well site, the cement casing on this rig was more than 10 feet deep and more than 1 inches wide without proper covering. The flooring was not securely held in place and employees were exposed to a fall hazard. The employee at this site fell thru the hole during a blast.

29 CFR 1910.132(e): Defective and damaged equipment. Defective or damaged personal protective equipment shall not be used. At this well site, on the rig floor, body belts observed were damaged and threading was frayed apart. Employees were exposed to a fall hazard or caught in between hazard while using this personal protective equipment when using to lift employees to an elevated height.

29 CFR 1910.184(f)(5)(iii): Wire rope sling(s) that had kinking, crushing, bird caging or other damage resulting in distortion of the wire rope structure were not immediately removed from service: At this well site, and times thereto, on the rig, wire rope was regularly used to lift heavy equipment from ground level. The wire rope had damage from crushing and the structure was distorted and was not immediately removed from service. Employees were exposed to a struck by and caught in between hazard.
