29 CFR 1910.1200(h)(1): Employer had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    (a)LaSalle Rental Properties One LLC, dba LRP One Trucking LLC at Nelson Farms Well site, Highway 257 and Weld County Road 78, Windsor, CO: On and preceding 4/21/15, LaSalle Rental Properties One LLC had not provided employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area.  Hazardous chemicals located in the workplace included, but are not limited to the following:    1.PfP Technology LLC LGA-54 fracking gel (containing petroleum distillates);  2.Chevron Products Company Chevron DELO Extended Life Pre-diluted 50/50 Coolant/Antifreeze PG (containing 1,2-propanediol); and  3.Unimin Corporation fracking sand containing crystalline silica).

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d):  (a)LaSalle Rental Properties One LLC, dba LRP One Trucking LLC, at Nelson Farms Well site, Highway 257 and Weld County Road 78, Windsor, CO: On and preceding 4/21/15, LaSalle Rental Properties One LLC did not verify that the required workplace hazard assessment had been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d).  Employees conducted operations which included exposure to hazardous chemicals.  Hazardous operations included, but are not limited to the following:  1.Exposure to PfP Technology LLC LGA-54 fracking gel (containing petroleum distillates) when cleaning up spills; 2.Exposure to Chevron Products Company Chevron DELO Extended Life Pre-diluted 50/50 Coolant/Antifreeze PG (containing 1,2-propanediol) when cleaning up spills; and 3.Exposure to Unimin Corporation fracking sand containing crystalline silica) when cleaning up spills.
