29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: (Construction Reference: 1926.59) (a) On or about 3/27/15 at 1283 York Avenue, employees pouring concrete "rat slab" were exposed to thermal and chemical burn hazards. The employer failed to ensure that its employee hazard communication training included specific work practices, emergency procedures, and personal protective equipment to be used to effectively protect from concrete burns. Training deficiencies included but were not limited to: (1) wearing rain gear and seam taping rubber boots, (2) instruction to remove sloughed concrete immediately from skin with water, and (3) instruction to seek medical attention at the first sensation of burning occurs.

Employees were exposed to thermal and chemical burn hazards, which resulted in injuries. The employer failed to ensure that frequent and regular inspections were made of the site, such that severe concrete burns were allowed to occur.
