29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employees:   a) PMA Plant - The employer failed to provide self-contained breathing apparatus, ( SCBA) to employees working on top of the Blend Tanks 1 and 2 when adding bags of Butaphalt -720 cross-linker to liquid asphalt manually. Employees were exposed to unknown concentration of hydrogen sulfide.

29 CFR 1910.1000(b)(2): Employee(s) were exposed to an airborne concentration of Hydrogen Sulfide listed in Table Z-2 in excess of the ceiling concentration of 3.8 microgram/ml for thiosulfate: (Thiosulfate is one the major metabolites of sulfide that is commonly in blood, therefore is the compound tested in Hydrogen Sulfide exposure)   A) PMA Plant - An employee was exposed to hydrogen sulfide in excess of the ceiling concentration of 3.8 microgram/ml for thiosulfate when working on top the Blend Tanks 1&2 and dropping bags of Butaphalt-720 cross-linker with ingredient of hydrogen sulfide into the  Blend Tanks 1&2. Elevated levels of thiosulfate (3.8 microgram/ml) were found.
