OSH ACT of 1970 Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to hazards of being caught-in / between the ingoing nip points of the DBCS intake belts / guide plate and the DBCS shorting belts / pulleys.  The below listed conveyor or systems were not guarded to prevent ingoing nip points:  a) On or about April 23, 2015 and times prior; DBCS Delivery Bar Code Sorter, intake/feeder position: The employer failed to ensure maintenance personal were protected from the in-going nip-point of the intake/feeder conveyor belts and the stationary guide plate during set-up procedures when hand feeding material.  b) On or about April 23, 2015 and times prior; DBCS Delivery Bar Code Sorter, sorting position: The employer failed to ensure employees working the shorter position were protected from the in-going nip-point of the feed conveyor belts and the equipment during the retrieval of mail while using their hands.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:  a) On or about April 23, 2015 and times prior; fixed stairs at trash compactor: The employer failed to ensure the stairs were constructed with the rise height being uniform throughout the flight of stairs exposing employees to the hazards associated with falling from the stairs.
