29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a) At Lot # 167, 1207 Silverbrook Court, Powder Springs, GA: The employer exposed an employee installing siding from a working platform of an Alum-A-Pole pump jack scaffold, to falls of approximately 20 feet 6 inches above ground level. The employee was not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed.    b) At Lot # 167, 1207 Silverbrook Court, Powder Springs, GA: The employer exposed employees installing siding from a working platform of a ladder jack scaffold, to falls of approximately 20 feet 6 inches above ground level. The employees were not wearing proper personal fall arrest system and neither guard rails or safety nets had been installed.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    a)  Lot # 167, 1207 Silverbrook Court, Powder Springs, GA: An employee installing siding, was exposed to a lower level fall hazard of approximately 20 feet 6 inches while working from an Alum-A-Pole pump jack scaffold that did not have its poles secured at the bottom.

29 CFR 1926.1053(b)(1):  A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Lot # 167, 1207 Silverbrook Court, Powder Springs, GA: The employer was using an extension ladder to access the roof of a single family house that was not extended three (3) feet above the landing exposing the employees to fall hazards.
