29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)ICO Construction LLC at 3485 E. 143 Ave., Thornton, CO 80023:  On and before April 24th, 2015, the employer did not ensure that an employee was wearing safety glasses when using a pneumatic nail gun when sheathing this roof. The condition exposed this employees to the hazard of projectiles to the eyes.

29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity:    (a)ICO Construction LLC at 3485 E. 143 Ave., Thornton, CO 80023:  On and before April 24th, 2015, the employer allowed an orange extension cord with a replacement plug having reversed polarity to be used with a 120 volt ungrounded Skilsaw.  The condition exposed employees to an electrical shock hazard.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection:  (a)ICO Construction LLC at 3485 E. 143 Ave., Thornton, CO 80023:  On and before April 24th, 2015, the employer did not ensure that employees were using a working ground-fault circuit interrupter (GFCI) when using an ungrounded circular Sklisaw with a flexible orange extension cord having reversed polarity.  The condition exposed employees to an electrical shock hazard.
