29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 4/24/15 at the Krones Labeling machine; employer failed to protect employees from caught between hazards while cleaning the label paddles without a developed and documented control of hazardous energy procedure. Two employees were involved in cleaning the Krones labeler, when one of the employees cycled the machine at the same time the other employee reached into the machine resulting in a caught between injury. b) On or about 05/5/15 on the Drop Packer for the plastic bottling line; employer failed to protect employees from caught between hazards when an employee reached into the drop packer to clean a jam without a developed and documented control of hazardous energy procedure. c) On or about 05/5/15 at the Case Stacker for the glass line; employer failed to protect employee from caught between hazards during cleaning of spills and jams of the case stacker without a developed and documented control of hazardous energy procedure. d) On or about 05/5/15 at the Palletizer for the plastic bottling line; employer failed to protect employees from caught between hazards during the cleaning of spills and jams of this machine with a developed and documented control of hazardous energy procedure. ABATEMENT DOCUMENTATION REQUIRED
