29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations a) During demolition a collapse of two floors and the outer wall occurred. Steel beam shelves were damaged and failed causing the 4th floor to collapse as mechanical equipment was removing concrete from the beam. Location: 622 W 57th Street, New York, N.Y. On or about 2/25/2015 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
