29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Production Floor - On or about April 28, 2015, an employee preparing the All-Case Furnace for service did not, or ensure that, an orderly shutdown of the machine was utilized to avoid any additional or increased hazards to employees as required in 29 CFR 1910.147(d)(2). (b) Production Floor - On or about April 28, 2015, all energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine from the energy source(s) as required in 29 CFR 1910.147(d)(3), in that the equipment was still in an energized state when preparatory work commenced. (c) Production Floor - On or about April 28, 2015, lockout or tagout devices were not affixed to each energy isolating device on the All-Case Furnace by authorized employees as required in 29 CFR 1910.147(d)(4)(i).
