Employees were exposed to fall hazard, electric shock hazard when doing exterior masonry repairs on the 30 story apartment building.       Location: 140 West End Ave, New York, N.Y. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which is 6 feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a) Employees were exposed to fall hazard doing masonry repair work on the 17th floor balcony.  The employer removed all of the balcony railings as part of the masonry repairs to the 30 story apartment building.       Location: 140 West End Ave, New York, N.Y.      On or about 3/31/2015               NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters (GFCI) or an assured grounding program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites from electric shock hazard when using electric tools and equipment:      a)  Employees were exposed to electric shock hazard using electric tools and equipment without ground fault circuit protection (GFI).  Masonry tools were connected to the blue extension cord which was plugged in to an power outlet on the scaffold. No GFI was being used.       Location: 140 West End Ave, New York, N.Y.      On or about 3/31/2015               NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

Masonry tools were connected to the blue extension cord which was plugged in to an ungrounded power outlet on the scaffold.      Location: 140 West End Ave, New York, N.Y.
