29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) Floor holes were not protected. There were two holes in the stairway used by employees to access the work areas. Employees can trip or catch their foot in a hole.    b) There was a 15 inch deep hole in the ground near the foot of the stairs. The unprotected hole was 2 ft wide and had a board placed over it which did not provide floor hole cover protection.     Location:  360 Neptune Ave, Brooklyn, N.Y.    On or about 4/1/2015    NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
