On 4/23/2015, workers from Utz Contracting LLC were repairing a roof of a building located at 115 New Ballwin Road, Ballwin MO. The owner and six employees were working on the roof, which was approximately 8 feet above the ground, without the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems.

On 4/23/2015, workers from Utz Contracting LLC were repairing a roof of a building located at 115 New Ballwin Road, Ballwin MO. They were using two ladders improperly to access the roof. The ladders were not extended to three feet beyond the landing surface or edge of the roof.
