29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:     a)  Gas cylinder storage area; On or about April 20, 2015 and at times prior thereto the employer exposed employees to fire hazards in that employees were allowed to walk and work in the same area where oxygen cylinders and fuel gas (acetylene) cylinders were stored in close proximity together without a barrier as required by the standard.

29 CFR 1910.217(e)(1)(i): Records of mechanical power press inspections and the maintenance work performed were not maintained:     a) Plant Floor; On or about April 20, 2015 and at times prior thereto the employer exposed employees to potential crushing and amputation hazards in that employees were alllowed to operate mechanical power presses that the employer had not maintained a certification record of required inspections and maintenance work performed.
