29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial construction site, employees performing framing activities on the roof without fall protection were exposed to fall hazards of approximately 23 feet to the concrete below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  At a commercial construction site, an employee performing framing activities connected to an anchor point approximately 12 feet below the working surface was exposed to falling more than 6 feet or to the concrete below.
