29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. (a) Building 1, 100 Business Center Drive, Cheswick, PA: On or about April 27, 2015 a standard guardrail was not in place around the loading area. The guardrail consisted of a cable top rail only and there were areas where the cable was slack and the height was below 42 inches. There was one section where there was no rail in place. Employees work in this area moving and loading large sheets of plate glass exposing them to potential fall hazards of 4 feet into the loading area below. (b) Building 2, 100 Business Center Drive, Cheswick, PA: On or about April 27, 2015 a standard guardrail was not in place around the loading area. The guardrail consisted of a cable top rail only and there were areas where the cable was slack and the height was below 42 inches. There were areas where there were no rails in place. Employees work in this area moving and loading large sheets of plate glass exposing them to potential fall hazards of 4 feet into the loading area below.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: (a) Facility, 100 Business Center Drive, Cheswick, PA: On or about April 27, 2015 the employer did not have a procedure to provide a level of protection for all persons during a group servicing and/or maintenance task. A group servicing task is performed by employees and a maintenance employee on the tempering line and only the maintenance employee applies a lock to control the energy.
