29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a) Production; On or about April 8, 2015 and at times prior thereto the employer exposed employees to fire hazards in that employees were allowed to work in areas where combustible dust had accumulated on the floors and equipment of the production area exceeding 1/4 inch in depth. b) Production; On or about April 8, 2015 and at times prior thereto, the employer exposed employees to slip, trip, and fall hazards due to a large accumulation of wood dust and miscellaneous scrap wood being placed in the work area and employee walkways.

29 CFR 1910.213(g)(1): Swing cutoff saw(s) were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw: a) Saw Mill; On or about April 8, 2015 and at times prior thereto, the employer exposed employees to amputation hazards in that employees were permitted to operate a chopsaw (swing saw) that did not have a guard that covered the lower portion of the blade.
