29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Restoration Hardware 255 Blair Road, Suite C, Avenel, NJ 07001 - Facility: The temporary agency employer did not develop, document and certify a hazard assessment to identify potential hazards temporary employees would be exposed to. Temporary employees were hired to work at Restoration Hardware where they were exposed to hazards associated with operations including but not limited to spray painting operations, and wood working machinery cleaning chemical reactors. A written certification of the hazard assessment was not documented. Condition noted on or about 4/27/2015.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Restoration Hardware 255 Blair Road, Suite C, Avenel, NJ 07001 - Medic Department: Employees were not administered medical evaluations to determine the employees ability to use a respirator before the employee was provided a 1/2 face elastomeric respirator (3M) for use during spray painting operations. Condition noted on or about 4/27/2015.
