On or about April 28, 2015 - at the above addressed jobsite, throughout the entire project (floors 4 - 15), floor holes, ranging in seizes of two (2) inch, four (4) inch, six (6) inch and twelve (12) inch diameter as well as an eighteen (18) inch by eighteen (18) inch square, were not provided with covers, thereby exposing employees and subcontractor employees to tripping in or stepping into or through the holes.

On or about April 28, 2015 - at the above addressed jobsite, throughout the entire project (floors 4 - 15) the plywood pieces used to cover the floor holes were not secured as to prevent accidental displacement, thereby exposing employees and subcontractor employees to trip hazards.
